Payment FAQ

What are your rates?

55-minute Initial Intake/Evaluation Session $175
55-minute Therapy Session $150

This is my full rate and I will bill insurance for you. Clients are responsible for the co-pay, co-insurance, or full cost of service if insurance deductible has not been met. Payment is expected at time of service. Clients are responsible for understanding their insurance coverage.

What types of payment do you accept?

Checks can be made payable to Montana Mental Health. Credit card payments can be made prior to session via my client portal or at the time of service. Credit card payments will incur a 3% service fee.

What is your cancelation policy?

Please notify me at least 24 hours in advance of your scheduled appointment if you need to cancel or reschedule. You will be responsible for the full fee for the session if you do not show for your appointment or if you cancel with less than 24 hours notice. The fee includes any co-pays and full amount insurance usually pays. Insurance does not reimburse for missed sessions. I understand that emergencies do arise. Please let me know as soon as possible if you are not able to make it to your appointment.

Do you take my insurance?

I am a preferred provider on the following insurance panels:

  • Blue Cross Blue Shield

  • Montana Health Co-op

  • PacificSource Health Plan

  • Cigna

  • Allegiance

  • Anthem

  • Out of network: For out-of-network insurance providers, I can provide you with an invoice to submit to your insurance, HSA, or FSA for reimbursement. Check with your insurance for out-of-network benefits.

What should I ask my insurance provider to see if services are covered?

Please check your insurance coverage carefully by asking the following questions:

  • What is my mental health deductible and has it been met? If not, how much is left to pay?

  • What is my start and renewal date?

  • What is my copay or co-insurance per session for CPT codes 90791 and 90837?

  • How many sessions per year does my health insurance cover?

  • Do I have mental health insurance benefits that pay out of network?
